This one-hour online Personalised Care Planning course introduces the principles and process of developing care plans that reflect an individual’s needs, preferences and goals. It explores the roles of health and social care staff, self-care, information prescriptions, collaborative goal-setting and effective communication. The course carries 1 CPD point and is designed to support awareness and good practice in health and social care settings.

A hard copy can also be requested. Unlimited quiz retakes are available.
