Verification of Death Course

This course is specifically designed to equip persons working in primary care, supportive care, and care homes with the knowledge and competency to formally undertake the Verification of Death (VoD) in the United Kingdom. This competence is critical, as it allows nurses to confirm the patient’s death quickly, providing dignity to the deceased and allowing the family to proceed with arrangements sooner.

Target Audience and Scope (UK Settings)

• Primary Care: Community/District Nurses and General Practice Nurses managing patients receiving end-of-life care at home or in primary care settings.

• Supportive/Palliative Care: Nurses in hospices or specialist palliative care units.

• Care Homes: Nurses in residential and nursing homes where they are frequently the most senior qualified professional present

Core Distinction: Verification vs. Certification

A fundamental focus of the course is the difference between two distinct legal/clinical acts in the UK:

• Verification of Death (VoD): The clinical procedure performed by a nurse (or doctor) to confirm the irreversible cessation of life. This course trains nurses in this procedure.

• Certification of Death: The legal procedure of issuing the Medical Certificate of Cause of Death (MCCD), which can only be completed by a doctor or, in Scotland, a doctor or a specialist nurse/paramedic registered to do so.

Learning Outcomes Detailed (UK Context)

1. Understand the Legal and Professional Context

• Legal: Covers the specific professional directives and local trust/Integrated Care Board (ICB) policies that permit RNs to verify death. It addresses the Nursing and Midwifery Council (NMC) Code of Conduct and accountability when performing VoD.

• Coroner/Procurator Fiscal: Understanding when a death must be reported to the Coroner (England/Wales/NI) or the Procurator Fiscal (Scotland)—a mandatory step for most unexpected or unexplained deaths.

• Documentation: Focus on the legal requirement for meticulous and contemporaneous record-keeping on all verification checks and subsequent communications.

2. Differentiate between an Expected and an Unexpected Death

The procedure, and the nurse’s immediate actions, vary significantly:

Context	Expected Death (e.g., terminal illness)	Unexpected/Sudden Death
Criteria	Death was anticipated, the patient was known to be dying, and the GP or medical team has been involved.	Death was sudden, unexplained, due to trauma, or occurring within 24 hours of hospital admission.
Nurse’s Action	Proceed with Verification of Death based on protocol. Follow the end-of-life care pathway documentation.	Do NOT proceed with Verification initially. Immediately inform the Police and report to the Coroner/Procurator Fiscal as the scene may become a crime or investigation site.
Policies	Adherence to local Palliative Care Policies (e.g., checking for the presence of a Do Not Attempt Cardiopulmonary Resuscitation (DNACPR) form).	Adherence to procedures for Preservation of the Scene and avoiding disturbance of the body or its surroundings.

3. Develop Knowledge, Skill and Competence to Sensitively Perform Verification of Death

This outcome focuses on the clinical and compassionate execution of the procedure.

• Skill: The Clinical Procedure

  • Systematic examination to confirm the irreversible cessation of cardiorespiratory function. This involves a continuous assessment (usually for a minimum of five minutes) of:

    • Absence of heart sounds (auscultation).

    • Absence of peripheral and central pulse.

    • Absence of breath sounds (auscultation) and respiratory effort.

    • Fixed and dilated pupils with no reaction to light.

• Sensitivity and Communication: Training in how to compassionately communicate the findings to the bereaved family, including using appropriate and clear language.

• Post-Verification Care: Understanding the logistical steps, such as contacting the GP/Medical Team to discuss the MCCD and liaising with Funeral Directors or mortuary services. This includes ensuring all medical lines/equipment (e.g., central lines, catheters) are dealt with appropriately and with dignity, following infection control policies.
